Ten Ten Ten Awards Provisional Certification to Cara AI for Home-Based Health Assessments

Multimodal AI platform recognized for reducing assessment costs, accelerating completion, and expanding access for Medicare and Medicaid populations

PITTSBURGH - PennZone -- Ten Ten Ten, a nonprofit improving U.S. health outcomes while reducing healthcare costs, today announced that it awarded Provisional Certification to Cara AI, a multimodal platform for home-based health and functional assessments.

The certification covers Cara's Long-Term Services and Supports functional assessments as the primary scope and its Health Risk Assessment module as a secondary scope. Cara enables patients and caregivers to complete assessments through voice, text, web, and guided photo or video review. The platform identifies mobility, medication, home-safety, and daily-living risks and converts them into structured documentation for care teams.

Ten Ten Ten Certification evaluates healthcare solutions across three pillars: Health Outcomes, Economic Benefits, and Equity. Provisional Certification is intended for emerging solutions that present credible preliminary evidence, disclose what remains unproven, and commit to generating confirmatory real-world data.

"Provisional certification does not mean every projected outcome has been proven," said Bhargav Raman, MD, MBA, cofounder and Certification Lead at Ten Ten Ten. "It means the solution has presented credible evidence, clearly disclosed what remains to be validated, and has a responsible plan for generating the evidence needed for full certification. Cara met that standard."

Ten Ten Ten found that Cara satisfied standards for Economic Benefits and Equity and met the provisional pathway for Health Outcomes. Evidence indicated the platform can maintain assessment quality while lowering costs, accelerating turnaround, increasing completion, and expanding coordinator capacity. Cara's multilingual, SMS-first, app-free design may also improve access for dual-eligible, rural, limited-English, and low-broadband populations.

"Too many patients experience delays or miss assessments because traditional in-home models are constrained by geography, language, staffing, and cost," said Renee Dua, MD, founder and CEO of Cara AI. "Cara was built to make assessments more accessible and consistent while giving care teams timely information they can use to coordinate care."

Longer-term outcomes, including reductions in emergency department use, hospital admissions, skilled-nursing utilization, and total cost of care, have not yet been independently demonstrated. Cara has committed to a confirmatory study using an intervention group and concurrent risk-matched control group.

Cara will complete annual re-attestation and report material changes to its pricing, ownership, data use, or business model.
