This innovative offering will provide post-admission check-ins, visit utilization mapping, end-of-life education, and more, creating additional touchpoints for patients and their families.

The recently launched patient engagement service utilizes a comprehensive approach to connecting with patients and their families beyond routine home visits. By working with providers to tailor care plans to individual needs and offering consistent nurse support, the service fosters stronger patient relationships and deeper insight into patient status. As with all IntellaTriage services, the foundation of the patient engagement service lies in its highly trained nursing staff. IntellaTriage employs over 300 licensed nurses, all with post-acute bedside experience. These nurses undergo robust internal training programs that prepare them for remote phone support, equipping the company with a uniquely skilled team to deliver this new service effectively. The new service line also aligns with ongoing and recent Centers for Medicare & Medicaid Services (CMS) quality care initiatives that focus on outcomes and overall care coordination, such as the home health value-based payment model and the upcoming HOPE Assessment tool for hospice.
